Juni 2021. Besondere Umstände erfordern besondere Massnahmen – und so freuen wir uns, dass wir den für Juni 2020 sorgfältig vorbereiteten Kongress aufgrund der anhaltenden COVID-19- Pandemie nun zumindest als virtuelle Tagung durchführen können. Die gegenwärtige Pandemie lehrt uns dreierlei. Erstens ist die Medizin durch einen stetigen Lernprozess geprägt, der die Entwicklung in einer sich fortlaufend verändern- den Welt sichert. Dass dabei auch bestehende Lehrmeinungen umgestürzt werden und neue Erkenntnisse zu Richtungsänderungen führen, liegt in der Natur der Sache. Zwei- tens bringt die Pandemie die Komplexität der medizinischen, gesellschaftlichen und ökologischen Zusammenhänge zum Bewusstsein. Gerade bei komplexen Herausforde- rungen führt aber oft nur die Berücksichtigung verschiedener Perspektiven zu tragfähi- gen und nachhaltigen Lösungen. Und drittens belebt die aktuelle Krise die Grundfrage nach der Entstehung von Gesundheit und Krankheit. Da beide mit dem Menschsein auf innigste Weise verknüpft sind, muss eine patienten-zentrierte Medizin von dieser Frage ausgehen und die Arzt-Patienten-Beziehung als Herzstück der Medizin von der Sorge um den notleidenden Mitmenschen bestimmt werden. In der täglichen Praxis begegnet die Kinderärztin/der Kinderarzt häufig der Frage: «Doktor, warum ist mein Kind krank?» Diese Frage lenkt den Blick auf die krankma- chenden Faktoren und damit auf die Pathogenese des Krankheitsgeschehens. Spätes- tens wenn die Diagnose gestellt ist, schliesst sich meist eine weitere Frage an: «Doktor, was macht mein Kind (wieder) gesund?» Dabei richtet sich der Blick neben den kon- kreten Therapiemöglichkeiten auch auf die gesundmachenden Faktoren und Entwick- lungsbedingungen, also auf die Salutogenese. Diese spielt in jedem Fall eine zentrale Rolle, ganz gleich ob die Therapie sogenannt kausal oder nur symptomatisch ist. Es sind diese beiden gleichberechtigten, sich ergänzenden Blickrichtungen, die auch der Unterscheidung von Risikofaktoren und Schutzfaktoren für die gesunde Entwicklung von Kindern und Jugendlichen zugrunde liegt. Den enormen Fortschritten der konventionellen Medizin verdankt man die grossartigen Erfolge z.B. in der Notfall- und Intensivmedizin oder der Onkologie. Daneben sind es insbesondere die Therapiemethoden der Komplementärmedizin, die gezielt versuchen, gesundmachende bzw. gesundheitserhaltende Prozesse anzuregen und zu unterstüt- zen. Obwohl sich komplementärmedizinische Massnahmen allgemein einer grossen Beliebtheit erfreuen, sind die Möglichkeiten der Komplementärmedizin gerade in der Kinder- und Jugendmedizin bei weitem nicht ausgeschöpft und zudem nicht ausrei- chend erforscht, um klar zwischen wirksamen und sicheren bzw. nicht-wirksamen und gefährlichen Therapien unterscheiden zu können. In einer kürzlich publizierten Studie zeigt sich ausserdem, dass zwei Drittel der Schweizer Pädiater Fortbildungen im Bereich der Komplementärmedizin wünschen, um ihre Patienten diesbezüglich kom- petent beraten zu können. 3 BEGRÜSSUNG So wie sich die beiden oben genannten Fragen sinnvoll ergänzen und erst gemeinsam einen ganzheitlichen Weg der Patientenbetreuung beschreiben, so können sich die Ansätze der konventionellen und komplementären Medizin ergänzen, will man das therapeutische Potenzial im Sinne der integrativen Medizin voll ausschöpfen. Der dafür nötige Dialog zwischen Praxispädiatern, pädiatrischen Spezialisten, Fachexper- ten und Forschern steht ganz im Zentrum der Jahrestagung 2021. Sie steht unter dem Motto «Building bridges between conventional and complementary medicine» . Les circonstances particulières appellent des mesures particulières – et nous sommes donc heureux que malgré la pandémie persis- tante de COVID-19, nous puissions tout de même organiser le congrès soigneusement préparé pour juin 2020 sous forme d’un congrès virtuel. La pandémie actuelle nous enseigne trois choses. Tout d‘abord, la médecine se carac- térise par un processus d‘apprentissage permanent, qui assure le développement dans un monde en constante évolution. Il est dans la nature des choses que des doctrines existantes soient renversées et que de nouvelles découvertes conduisent à des chan- gements d‘orientation. Deuxièmement, la pandémie met en lumière la complexité des interrelations médicales, sociales et écologiques. Cependant, surtout lorsqu‘il s‘agit de défis complexes, ce n‘est souvent qu‘en considérant différentes perspectives que l‘on peut trouver des solutions viables et durables. Et troisièmement, la crise actuelle ravive la question fondamentale des origines de la santé et de la maladie. Comme ces deux éléments sont intimement liés à l‘être humain, une médecine centrée sur le patient doit partir de cette question et la relation médecin-patient, qui est au cœur de la médecine, doit être déterminée par le souci du prochain dans le besoin. Dans la pratique quotidienne, la/le pédiatre est souvent confronté/e à la question: «Docteur, pourquoi mon enfant est-il malade?» Cette question oriente le regard vers les facteurs pathogènes et donc à la pathogenèse de la maladie. Et plus tard, lorsque le diagnostic est posé, une autre question s’ajoute: «Docteur, qu’est-ce qui guérit mon enfant?» Ici l’attention est dirigée vers les mesures thérapeutiques concrètes ainsi que les facteurs et conditions de développement nécessaires au rétablissement de la santé, à savoir la salutogenèse. Celle-ci joue de toute manière un rôle primordial, que ce soit lors d’une thérapie causale ou uniquement symptomatique. Ce sont ces deux visions équivalentes et complémentaires, qui sont aussi la base pour la distinction entre fac- teurs de risque et facteurs de protection pour le bon développement des enfants et adolescents. Grâce aux progrès énormes de la médecine conventionnelle, de grands succès ont été atteints p.ex. en médecine d’urgence, en soins intensifs ou en oncologie. D’autre part, ce sont notamment les mesures thérapeutiques de la médecine complémentaire, qui visent spécifiquement à stimuler et soutenir les processus de maintien et de rétablisse- ment de la santé. Bien que les thérapies de la médecine complémentaire soient géné- ralement très appréciées, leurs possibilités sont loin d’être épuisées en pédiatrie et ne sont pas suffisamment étudiées pour distinguer entre des thérapies efficaces et sans risque, et des thérapies inefficaces et dangereuses. De plus, dans une enquête publiée récemment, deux tiers des pédiatres suisses souhaitent suivre des formations de mé- decine complémentaire, pour mieux conseiller leurs patients à ce sujet. Comme les deux questions susmentionnées se complètent judicieusement et per- mettent seulement ensemble une prise en charge globale du patient, les approches 5 MESSAGE DE BIENVENUE des médecines conventionnelle et complémentaire doivent également se compléter, afin d’exploiter entièrement le potentiel thérapeutique d’une médecine intégrative. Le dialogue nécessaire entre les pédiatres en cabinet, les spécialistes pédiatriques, les experts et les chercheurs scientifiques est au centre du congrès 2021. Special circumstances require special measures – and so we are pleased that, due to the ongoing COVID-19 pandemic, we can now hold the congress carefully prepared for June 2020 at least as a virtual meeting. The current pandemic teaches us three things. First, medicine is characterized by a continuous learning process that ensures development in a constantly changing world. It is in the nature of things that existing doctrines are overturned and new findings lead to changes in direction. Secondly, the pandemic brings to light the complexity of medical, social and ecological interrelationships. However, especially with complex challenges, often only the consideration of different perspectives leads to viable and sustainable solutions. And third, the current crisis revives the fundamental question of the origins of health and disease. Since both are intimately linked to the human being, a patient-centered medicine must start from this question and the doctor-patient re- lationship, as the heart of medicine, must be determined by the care for the suffering fellow human being. In daily practice, the paediatrician is often confronted with the question: «Doctor, why is my child ill?» This question directs attention to the disease-causing factors and thus to the pathogenesis of the disease. As soon as the diagnosis has been mad\ e, usually another question follows: «Doctor, what will make my child healthy (again)?» Aside from specific therapies, this question draws attention to health promotion factors and deve- lopmental conditions and, therefore, to salutogenesis. This approach plays an important role, whether the therapy is causal or symptomatic. These equal and complementary perspectives are also the basis for the distinction between risk factors and protective factors for the healthy development of children and adolescents. With the enormous progress that has been made in conventional medicine, major suc- cesses have occurred in fields such as emergency medicine, intensive care and onco- logy. At the same time, complementary medicine therapies have strived to stimulate and support the promotion of health and the processes that maintain it. Although com- plementary medicine enjoys great popularity, its therapeutic potential is far from being used to the extent possible in paediatrics, nor is it being investigated scientifically to allow clear distinctions between effective and safe therapies and ineffective and dan- gerous therapies. Furthermore, according to a recently published study, two thirds of Swiss paediatricians are interested in training courses on complementary medicine to provide appropriate advice to their patients on this issue. Just as the two questions posed by parents complement each other reasonably, together describing integrated patient care, the approaches of conventional and complementary medicine can together complement one another to use the entire range of therapeutic potential according to the principles of integrative medicine.
